All routine medications require 48 hrs notice to refill.

Appointments required if not seen in a year.

Payment is due at the time of service.
Please keep our office updated with changes to your insurance policy.
In the event your health plan determines a service to be "not covered", you will be responsible for the complete charge. Payment is due upon receipt of statement.

If you have insurance coverage with a plan we do NOT accept, total charges are due at time of service.
